Students will need a Physical, Cardiac Arrest Form, Concussion Form, Heat and Illness Form and Sportsmanship form all signed to participate in athletics for the 2025-2026 school year.

Forms must be completed prior to conditioning/tryouts.  You can find the links in the quick links section of this website also. 

PLEASE read the informational flyer in the information center about the new physical requirements.

No physical, concussion form, cardiac arrest form, heat and illness form or sportsmanship form = No practice, conditioning or tryouts, No exceptions.

Students with medical needs such as inhalers and epipens must have them present at all practices, conditioning and games.
